Unplanned hospital readmission has been widely used internationally as an indicator of quality and safety of health care. By providing appropriate care along with good discharge planning, we aim to reduce the likelihood of unplanned hospital readmissions. We ensure patients are discharged with care instructions they need after a hospital stay and help them recognise symptoms that may require medical attention. |
